MEND Employee Insurance Benefits

A MEND group insurance plan is available to full-time employees and their eligible dependents.
MEND pays the entire medical premium for the employee. Employees may elect to add coverage for eligible dependents at their own cost. Additionally, MEND covers 50% of dental and vision premiums for employee only if employee chooses to elect this coverage. Coverage is plan specific and will be discussed with employee. Optional upgrades may be available at the employee cost.
Complete details concerning this insurance will be given to you at the time of eligibility.
Eligibility for this program begins on the first day of the month following the completion of sixty (60) days of employment. Federal and state law (COBRA) provides that in case of termination of employment, or certai
n other events, an employee and his or her family members may be able to continue group insurance coverage by paying the full monthly premium themselves. Further information will be provided to you in case of termination of employment.
While it is MEND’s present intention to continue these benefits, MEND reserves the right to modify, curtail, reduce or eliminate any benefit, in whole or in part, either with or without notice. We recognize our responsibility to provide you reasonable notice of all material changes that may affect you.
